Digital waivers and consent forms, signed on the customer's phone
Clipboards and paper waivers slow down the front desk, get lost, and are hard to find when you need them. With Waitlist App you write your waiver, consent form or release once, customers sign it on their own phone with no account to create, and every signature is kept with the exact wording they agreed to. You can also make it required, so nobody books, joins or pays without signing first.
What it does
- Write it once. Create a waiver, consent form or release in plain text. Placeholders fill in the customer's name, your business name and the date for each person.
- Signed on their phone. Customers open a link in their normal phone browser and sign by typing their name, ticking an acknowledgement, or both. No app and no account.
- Required before they book or pay. Make a form required and customers sign it before a booking, joining your line from your storefront, a membership checkout, or paying for tickets and passes. Each person signs once and is not asked again.
- A record you can rely on. Every signature stores the exact text that was signed, a fingerprint of that text, the signer's name and email, the time, the IP address and the device. Signatures cannot be edited or deleted.
- Edits never change what was signed. Update your template whenever you like. Documents already signed keep the wording the customer agreed to.
- In your customer's language. The signing page appears in your storefront's language.
How customers get the form
A personal signing link
Create a signing link for a named client from your console and send it the way you already talk to them: text, email or chat.
A self-serve link for everyone
Each form has a shareable link. Customers enter their email and sign. Put it in your booking confirmation, on your website, or on a sign at the counter.
On your storefront
Your storefront can show a "Forms to complete" checklist, so customers sign ahead of their visit and see a tick when they are done.
As a step in booking or checkout
Required forms appear as a step when someone books, joins your line from your storefront, buys a membership or pays for tickets, so nobody gets through without signing.
Who uses it
Tattoo and piercing studios
Get consent and release forms signed before the client sits in the chair, with the signed copy on file under the client's profile.
Spas, salons and treatment rooms
Collect consent for treatments ahead of the appointment instead of on a clipboard at the desk.
Gyms, studios and classes
Make a liability waiver part of membership checkout so every member has signed before their first class.
Tours, rentals and events
Require a release when customers book or buy tickets, so the paperwork is done before anyone arrives.
What the record holds
Open any signed document to see the text exactly as it was signed, who signed it, when, and from which IP address, and print it or save it as a PDF from your browser. Each customer's profile lists the documents they have signed, and each form lists everyone who signed it. A customer can also send a form back with a reason instead of signing.
Free vs paid
Start with a free account to run your line and storefront. Waivers and consent forms are included with Pro, and on Enterprise our team works with you to tailor your documents. Current plans and prices are on the pricing page.
Waiver and consent form FAQ
Do customers need an account or an app?
No. They open a link in their phone's browser and sign there.
How do customers sign?
By typing their full name, ticking an acknowledgement box, or both. You choose for each form. Before signing they see a short statement that they are agreeing to sign electronically and that their signature, the date and their device details are recorded.
Can I require a waiver before someone books?
Yes. A required form becomes a step before booking, joining your line from your storefront, buying a membership, or paying for tickets and passes. Someone who has already signed is not asked again.
What if I change the wording later?
Change it any time. Documents already signed keep the exact text the customer agreed to.
Where do I see who has signed?
In your console. Each form lists every signature, each customer's profile shows their signed documents, and any signed document can be printed or saved as a PDF.
